Most people that come to see a functional medicine physician are looking for someone to listen to their whole health journey and offer guidance to interpret diverse symptoms, experiences, genetics and labs to help them identify the root cause of illness. Often their goal is to work toward reversing disease symptoms and promoting longevity.

Dr. Leazenby has always had a special place in her heart for the care of people experiencing complex medical symptoms. Over the years, she realized that the traditional medical model is failing many of these patients because the traditional practice was too focused on individual body systems and not on the big picture.
Treating the whole person requires time and attention. She has been told over and over by struggling patients that no one would listen... that no one understood... that no one knew where to start. There’s just no way to adequately evaluate, educate, encourage and equip a person struggling with physical or emotional discomfort in a 15 minute time slot a few times a year. At Relish Health, Dr. Leazenby wants to provide a comprehensive approach to health care that treats the whole person.

After years of experience in primary care and functional medicine, Dr. Leazenby believes a model exists for the care of patients that will help improve quality of life. This integrative model will provide more time, attention, empathy and expertise to patients.

As a functional medicine physician, Dr. Leazenby works with people who are chronically ill as well as those seeking optimal wellness. She works with each patient to identify patterns and connections that may be leading to the body’s imbalances and expression of disease. Dr. Leazenby spends considerable time getting to know you and your health history in order to understand the factors that have contributed to your current condition. Once identified, she works with you to develop a treatment plan to create health and balance, often beginning with nutrition, exercise and self-care skills. When necessary, these foundational skills are combined with medication, limited targeted supplements and outside professional consultation. For a functional medicine physician and their patient, finding a diagnosis is the beginning, not the end. The goal is to determine the "why" of your health issues and then work to restore balance and functionality to your body.

Dr. Leazenby specializes in:

  • Hormone management for women in pre-menopause, peri-menopause and menopause including hormone replacement therapy.

  • Weight management. Dr. Leazenby is fellowship trained in Obesity Medicine. She uses holistic functional medicine approaches with conventional medications to optimize your weight journey.

  • Gut health to reduce symptoms of constipation, diarrhea, bloating, reflux and irritable bowel syndrome through lifestyle, diet and targeted supplements or medications.

  • Metabolic health to keep your heart and mind healthy through optimization of blood sugar, blood pressure, and cholesterol.

  • Longevity support. Age well by optimizing and tracking hormones, metabolic markers, and keeping inflammation low.

LABORATORY SERVICES

As part of your healing plan, laboratory studies may be ordered to further identify causes of your illness, discover genetic predispositions, or to evaluate your response to therapy you are already receiving. Specimens are collected at a nearby facility, or in some cases in your home. Labs are reviewed at a follow-up visit to ensure results are used to collaborate with you to create a care plan that continues to put you in charge of your health.

Whenever possible, billing for these studies will be done by the particular lab and submitted to your insurance carrier. Be sure to check with your insurer before your visit so you are aware of what your particular plan covers and requires.

PAYMENTS AND INSURANCE

Dr. Leazenby is an “out-of-network” provider and does not contract with insurance companies including Medicare and Medicaid. Payment for office visits are expected at the time of service. Initial consultations are approximately 90 minutes in duration, and follow-up consultations are approximately 45 minutes in duration.

 At the end of each visit, you will receive a receipt detailing physician charges including specific insurance codes. You may submit this receipt to your insurance company for reimbursement as outlined in your plan documents. Because Dr. Leazenby does not participate in Medicare, patients should not and cannot file for Medicare reimbursement. Medicare patients will be asked to sign a form prior to a visit that acknowledges understanding of this policy. Patients may use pre-tax dollars from a Health Savings Account (HSA) to pay for office visits.

In addition to time spent in person with you, Dr. Leazenby spends considerable time in advance reviewing your information and preparing for your visit. As such, it is important to make any changes to scheduled appointments at least 48 hours in advance. A $50 fee will be charged for the first missed or late-canceled appointment. Subsequent missed or late-canceled appointments will be billed for the full amount of service scheduled.

Additional 15 minutes added to any visit: $80
InBody Body composition assessment outside of a regular appointment: $100

Payment is due at the time of service. Payment is accepted by credit card, cash or personal check. 



ONLINE PHARMACY

Relish Health has partnered with Fullscript on-line pharmacy to provide a quick and convenient way for patients to purchase vitamins and supplements. Dr. Leazenby may provide written recommendations for you at your office visit or she may send you her suggestions via email.
